New Patient Information

As a new patient to our practice, you will be given the opportunity to register and complete your medical history online prior to your first appointment. Whenever possible, please complete your online registration at least 24 hours prior to your appointment. 

When you make your appointment, we will ask for your email address plus other pertinent information. Once the appointment is made you will receive an email with an invitation to join the patient portal to complete your registration and history.

What do you do if you don’t have 24 hours or an email address? No problem, you can complete the registration and history in the office on the day of your appointment.

To better serve you, please arrive at our office at least fifteen minutes in advance of your first appointment. Once you come to the office for your first appointment, we will need to make a copy of your insurance card and your driver's license.  We will also collect your copay at that time.

We appreciate the opportunity to serve you and the confidence you have placed in our providers.  Always feel free to call our office with any questions at 540-361-1830.  You can make an appointment by pressing 1.  You can also leave a message for one of the providers or ask for a prescription refill all from the main menu of our phone system.
