For Your Appointment

If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:

  • Driver's License or State ID
  • Referral (If Required by Insurer)
  • Insurance Information
  • Copies of Operative Reports, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits regarding your current medical condition 
  • A List of Current Medications and Any Known Allergies
Our Policies


If you are unable to keep any appointment, please call us a day in advance so we can give that appointment time to another patient.  There is a $50 no show fee if you do not call to cancel your appointment twenty four (24) hours in advance. 

Fees & Payments

Copayments are expected at the time of service.  We accept cash, checks, debit cards, Mastercard and Visa.  We do all of our own claims processing and billing in house, so if you have a billing question, insurance question, or need to make payment arrangements you can call us at 540-361-1830, option 9.  We participate with many insurance companies and we will file your insurance claim for you. 

Prescriptions & Renewals

Prescription refill requests can be left in the prescription mailbox by dialing 540-361-1830, option 3.  Please leave your name, date of birth, the prescription you want refilled, the pharmacy name and location and your phone number.  You can also go to the contact us section of this website here and leave the information above in the form provided. Please leave your phone number so we can call you if we have questions.

** Prescription refills for our Orthopedic patients need to be called in forty-eight (48) hours in advance.

** Prescription refills for our Pain Management patients will need to be called in seventy-two (72) hours in advance. No prescription refills will be provided on Fridays or over the weekend.
